Couldn't Rand go into Tel Aran Rhiod in the flesh, imagine a new hand and cleanse his wounds then simply step out? Birgette's arrow seemed to materialize once it was thrown out of TAR, and while fighting Rhavin, Lews Therin tried taking control of Rand's body by trying to transform it, so I don't see why he couldn't "fix" his body like this.

Because the hand would be a construct of Tel'aran'rhiod, and those things can't come back out.  Birgitte's silver arrow ... is an odd exception, I'll admit, but it was most likely caught in Moghedien's specialized "ripping out weave".  Since Rand isn't a full-time occupant of T'A'R at the moment, that weave wouldn't work on him.

 

It may be possible that some kind of adaptation along the same lines as Moghedien's weave could have that kind of effect, but its certainly not just a matter of "Walk in, imagine a hand, walk out."

He doesn't have the time to do the research (not that he even knows about Moghedien and Birgitte), but I wouldn't say that he doesn't care. He's stone cold, but nobody's that cold.

He doesn't even think about it.  He is even more callous toward himself than he is toward anyone else ... thats part of his problem.  He has simply written off that hand within moments of losing it.

Yes, Rand is beyond cold.  He only has warmth for Min, Elayne, and Aviendha, and even that is rapidly diminishing.  His LOVE for them isn't waning, but his ability to show emotion even around them is nigh on impossible of late.

The "ripping out," was a bit different though, and quite unintentional.

That is when you are dreamwalking, like when they use Ter'angreals and all that jazz.  If you enter in the flesh I don't think that rule applies, but I also don't know what actually would happen.

Remember had those fishbites in tFoH after he was in T'A'R fighting Rhavin? They didn't fade immediately, but they did fade when Rand left T'A'R. Same principle. There is an RJ quote somewhere, but I'm lazy.

Another example are Egwene's bruises from the beating she took before leaving the Aiel.  They came back once she left Tel'aran'rhiod.
