In what book is it that Mat Died a second time, I have read all of the 6 times and I can not remember him dying a second time.

He has not yet died twice.  He died once, in The Fires of Heaven, in Caemlyn, but that death was undone when Rand balefired Rahvin, fulfilling the Aelfinn's statement that he would "die and live again, and live once more a part of what was".  He will, of course, die again someday, and that will be number two.

 

Some people think that he died when he was hung by the Eelfinn in Rhuidean.  This is not the case.  He came close, but was not actually dead, per Robert Jordan.

 

As purely a side note ... in the Structured Wheel of Time forum ... only moderators are supposed to start new threads.  For future reference, it would be a better idea to start this sort of thread in the General Wheel of Time forum.

Yup...now it has spoilers for anyone not past that point in book 5. Of course, the whole "Who Killed Asmodean?" thread isn't a spoiler in itself...
