No, he is not dead.

What he's been up to since he left Far Madding in WH...RAFO. My guess is mostly avoiding being hunted down by Slayer, possibly attempting to find a new base of power.

i think shadar logoth being destoried hurt him. more then likely he will be in AMOL striking at Rand as Rand striks at the DO

He was also possibly sending Trollocs and Myddraal through the Ways to kill Rand. Though that's still speculation at this point.

 

My guess is, even if he didn't send them, that he's massing some diabolical plan and will play a vital role in the Last Battle.

That was probably Taim and beside I don't he can control a number anywhere near that large.

Extraordinarily unlikely speculation.  Besides the fact that Fain can't control that many Myrddraal, or duplicate the effect of the Chosen mark, theres also the fact that he won't ever send anyone else to kill Rand.  He wants to kill Rand himself.
